Noticeboard

BOOST DAY is this Thursday 2nd December and is a day where you’ll see a mass media campaign takeover promoting the uptake of vaccines ahead of the festive season and there are a range of resources to support this on the Campaign Resource Centre (CRC). BOOST DAY will drive mass awareness and momentum around the importance of boosting your immunity this winter.

Messaging encourages those eligible to book their vaccines as soon as possible so we can all enjoy the festivities safely with our loved ones. This includes both flu and COVID-19 boosters, the evergreen offer for anyone still to get their 1st and 2nd doses of COVID-19 vaccine and second doses for 16-17-year olds. We welcome you all to get involved and use the BOOST DAY materials to make this day as big a success as possible.

Other new campaign material that you might have missed includes:

Winter Vaccinations
Covid-19 Health Behaviours
Hannukah Social Statics - For those who have not seen the new Festivals and Celebrations assets that went up on the 25th Nov - Hannukah runs until Dec 6 so there is still time to use these social media resources.
You can also find a helpful guide to navigating our Campaign Resource Centre (CRC).

Additional Registration Forms.pdf

Registration with the practice

If you wish to register with the practice you need to contact us - in person or by telephone - to make sure that you live in our catchment area.

The registration process itself is very simple.

When you visit the surgery for the first time you will need to show proof of address (anything with your name and address on it - like a phone / utility bill, tenancy agreement etc). We also accept registrations from people with no fixed address.

You will then be given a registration form to complete. Alternatively you can print off the registration form below, fill it out and bring it in with you.

The form asks for your NHS number. If you don't know it, you can contact your previous doctor. We need your NHS number for the electronic transfer of your medical records. We can register you without your NHS number but having this number will speed up your registration.

We will also ask you to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Again, you can print off the form below, fill it out and bring it with you.
Addional Registration Forms

Once you have completed the paperwork you will be given an appointment for a New Patient Health Check.

We routinely offer tests for HIV and Hepatitis to our patients as part of the New Patient Health Check.
